A forms search can be done using any one of the following:
- AOC Form Number – enter the AOC form number (i.e. AOC-J-120, J-120, or 120)
- Form Title – enter the Form title or a "keyword or phrase" in the title
- Category – select one of the categories from the drop down list
- For – select one of the categories from the drop down list

| A forms search can be qualified by using a combination of "keyword or phrase" entered in the Title field and selecting from the Category or For drop down lists. |
