The SelfServe Center website does not have forms for Absolute Divorce (Uncontested) or Name Changes. You may obtain these forms from the SelfServe Center located at 832 E. Fourth Street, Suite 3350, Charlotte, NC 28202 or through the ICAN website at www.icandocs.org/nc.
