For your convenience and benefit, the AOC offers the opportunity to receive payments electronically, rather than by check. Your payments will be automatically deposited into the checking or savings account of your choice. In attention to the electronic deposit, you will be automatically notified of the deposit, either by fax or by email.
An email notification can only be used if you have the capability to view attachments, otherwise you must use a fax notification. The fax or email will provide you with all the information that would normally be on your check stub. Please note, only one method of notification can be chosen. To receive payments electronically, you must print, complete this form, if applicable, attach a voided check and return both to the address listed on the form.
