Passports

The Clerk of Superior Court is the only agency in Sampson County with authority to execute passport applications. Passport applications for new or renewal applications are available in the Clerk's office at 101 E. Main St. or you may go to U. S. Department of State, Passport Information and download your application.

This is the only other application type that will be accepted. All applications need to be legible and in black or blue ink. ALL SECTIONS NEED TO BE COMPLETED. ALL APPLICATIONS SHOULD BE SIGNED IN THE PRESENCE OF A COURT CLERK.

The following items are required when applying for a passport.
  • A certified birth certificate or naturalization citizenship papers.
  • A picture ID for proper identification.
  • All applications must be accompanied by two, 2x2 identical photographs taken within the last six months with clear likeness (no shade glasses, hats, etc.) using a light background.
    • The image size from the bottom of the chin to the top of the head shall not be less than one inch or more than 1 3/8 inches.
    • The photographs may be black and white or in color.
Renewal applicants submit form DSP-82 together with their previous passport provided it was issued after their 16th birthday and issued within the last 15 years.

Applications for children under the age of 14 require both parents signatures.

Emergency applications require an urgent departure from the U.S. in less than 14 days, and/or require one or more Visas to be issued within a brief timeframe. In these cases, an executed passport application may be returned to the customer in a sealed envelope to be hand-carried, either personally or by a third party, to a Passport Agency. An appointment is still required to deliver the application to a Passport Agency. Hand-carried applications MUST include proof of departure, the $60.00 expedite fee and written authorization from the customer if a third-party is involved.

When paying passport fees, make your check payable to the US Department of State for $67.00 if 16 years or older with a $30.00 fee to the Clerk of Court. For applicants under 16 years of age, the US Department of State fee is $52.00 plus $30.00 to the Clerk of Court. If a person needs expedited service, there is an additional fee of $60.00 and you will need two overnight mailers with $16.25 postage attached to each mailer. This service usually has a 10 to 12 day turnaround time.
